Flu Vaccine Consent

F.A.O Parents/carers who will be in reception, year 1,2,3,4,5 and 6. The flu vaccine is being offered to your child. We must have your consent for the NHS team to deliver the vaccine which is given via a quick and simple spray up the nose. Please visit the link www.hdft.nhs.uk/electronic-consent or scan the below QR code to consent for each child. You will be asked to enter a secure school code which is:

The programme is in place to help protect your child against flu. Flu can be a very serious illness and sometimes causes major health complications particularly in children and the elderly. Vaccinating your child will also help protect more vulnerable friends and family by preventing the spread of flu. If you have any questions or do not have internet access please contact the team on 03000 032 554.
